The passing of a loved one can be one of the most difficult things we will ever face, and making the many decisions which come at a time of loss can be overwhelming.
Our cremation service would like to offer you support, care, and dignity during this time. Our cremation services has more than 20 years experience in helping families in their time of grief.
Step 1 – Notification
Call us and inform us of the passing. We will need to know where your loved one is located so we can respectfully take them into our care. If you are located in the Dallas Fort Worth area, please call us at 817-910-4497. If you are located in Amarillo, please call us at 806-425-2449. Rest assured, when you call we will walk you through the entire cremation process.

Cremation Package
Direct Cremation with Temporary Urn
- Transportation of your loved one to our crematory
- Cremation
- 1 copy of the Death Certificate

- Ashes returned in a temporary Urn (shipping available in the US)
Additional Death Certificates
Many entities require that an original death certificate be submitted for notification or to close accounts. 1 copy of the Death Certificate is included with the Cremation package. For additional copies, make your selection here.
Additional Shipping
The option for shipping is included with the Cremation package. Use this option only if the cremated remains need to be shipped to more than one destination.

Identification of Deceased Form
Please speak with a representative from Ashes to Ashes Cremation to determine if you need to download this form.
Dallas County Medical Examiner’s Office Release Form
This form is only necessary if your loved one is currently at the Dallas County ME’s office.
US Flag for Burial Purposes
This form is only necessary if your loved one was a member of the military and might be eligible for a US burial flag. Read instructions on page two of the form for submission options.
Visual Identification Acknowledgement Form
Please speak with a representative from Ashes to Ashes Cremation to determine if you need to download this form.
Tarrant County Medical Examiner’s Office Release Form
This form is only necessary if your loved one is currently at the Tarrant County ME’s office.
Military Discharge form DD214
This form is only necessary if your loved one was a member of the military and you need the records to access military benefits. Form will be filed on-line.
